Step-by-Step Process

Follow these steps to request your birth certificate

Check Eligibility

You can request if you are the owner, parent/guardian, or authorized representative with proper authorization letter.

Prepare Documents

Bring valid government-issued ID. If representative: authorization letter and IDs of both owner and representative.

Visit Civil Registrar

Go to the Local Civil Registrar Office at 2nd Floor, Municipal Hall. Office hours: Mon-Fri, 8AM-5PM.

Fill Out Request Form

Complete the request form with full name, date of birth, place of birth, parents' names, and purpose.

Pay the Fee

Pay ₱80 per certified copy (₱50 transcript + ₱30 DST) at the Municipal Treasurer's Office (same building).

Claim Certificate

Return to Civil Registrar with official receipt. Certificate released within 5-15 minutes if record is available.

Can I get a birth certificate registered in another municipality?

For local municipal copies, the Agoo Civil Registrar only issues certificates for births registered in Agoo. However, for births registered anywhere else in the Philippines, you can request national PSA copies directly at the Agoo Civil Registrar Office via BREQS.

What are the requirements for Delayed Registration of Birth in Agoo?

For delayed registration, submit: (1) PSA Negative Certification, (2) Affidavit of Delayed Registration, (3) Joint Affidavit of 2 Disinterested Persons, and (4) Two supporting public documents showing date/place of birth (e.g., Form 137, Baptismal Certificate, Voter's Record). Counter evaluation takes 13 minutes, followed by a mandatory 10-day posting period.

What's the difference between local copy and PSA copy?

Both are official certified true copies. PSA copies are printed on national security paper (SECPA) required for passports, visas, and DFA. Local copies (₱80) are issued directly on-site and accepted for local employment, school enrollment, and local government transactions.
